Titolo della tesi: VALUTAZIONE DEL SISTEMA UDITIVO-VESTIBOLARE NEI PAZIENTI AFFETTI DA APNEE OSTRUTTIVE DEL SONNO

The Ph.D. project aimed to evaluate the possible dysfunction of the vestibular system in patients with Obstructive Sleep Apnea (OSA) using the most recent diagnostic methods of vestibular diagnosis. Literature has reported an increased interest in this topic, although the current studies on OSA patients were incomplete and poorly standardized. The project's first phase aimed to select a population without comorbidities, evaluated using the fHIT (functional Head Impulse Test). This method has never been reported before for vestibular screening in OSA patients. The first published study showed a possible latent vestibular impairment, early diagnosed with fHIT. It could be due to the relationship between the vestibular nucleus (VN) and sleep regulatory pathways. Consequently, it was investigated if vestibular rehabilitation could modify the OSA severity score. The study was conducted on 20 patients without comorbidities. These preliminary data supported the thesis of a possible and strong connection between VN and sleep regulation patterns. The preliminary data reported an improvement in vestibular function and simultaneously in sleep apnea severity index. The probable explanation is that the vestibular stimulation increased the parabrachial nuclei's action and the orexinergic system's function. The project is proceeding with the comprehensive analyses of the five vestibular receptors performed by vestibular myogenic evoked potentials (Ocular VEMPS and Cervical VEMPS), vHIT (Visual Head Impulse Test), and fHIT. The results are encouraging, with an evident latent vestibular damage detectable by all methods. Further studies are undergoing in a more extensive case series of OSA subjects vs. controls, with a comprehensive vestibular screening (vHIT, fHIT, and VEMPS) to confirm the data. Moreover, audiological evaluations of the same subjects are underway. Finally, studies are in place to evaluate vestibular function after treatment of obstructive sleep apnea (CPAP, surgical therapy). Il Progetto di tesi svolto in questi tre anni di dottorato in “Tecnologie avanzate in chirurgia” ha avuto come obiettivo la valutazione del sistema vestibolare nei pazienti affetti da Apnea Ostruttiva del Sonno (OSA) sfruttando le metodiche diagnostiche più recenti. La letteratura in merito alla tematica sta facendo emergere un crescente interessamento nei confronti di questa tematica, sebbene ad oggi gli studi proposti risultassero frammentari nella valutazione sistema vestibolare in soggetti affetti da OSA. La prima fase del progetto ha avuto come obiettivo la selezione di una popolazione priva di comorbidità valutata mediante l’ausilio del fHIT (functional Head Impulse Test), metodica mai utilizzata per lo screening OSA in letteratura. Lo studio, attualmente pubblicato, ha dimostrato che i questi pazienti può essere presente un'alterazione vestibolare latente, diagnosticata precocemente con il fHIT. Una possibile spiegazione potrebbe dipendere dalla relazione tra il nucleo vestibolare (VN) e le vie di regolazione del sonno. Si è provveduto a valutare come una riabilitazione vestibolare potesse influire sulle OSA. Lo studio è stato condotto su 20 pazienti privi di comorbidità. Questi dati preliminari avvaloravano la tesi di una possibile e forte connessione tra VN e il pattern di regolazione del sonno. I dati preliminari, infatti riportavano un miglioramento della funzione vestibolare e contemporaneamente degli indici di severità delle apnee del sonno (AHI; ODI). Sembra probabile, pertanto, che la stimolazione vestibolare aumenti l'azione dei nuclei parabrachiali e la funzione del sistema orexinergico. Il progetto sta procedendo con la valutazione completa dei 5 recettori vestibolari eseguita mediante i potenziali evocati miogenici vestibolari (Ocular VEMPS e Cervical VEMPS), vHIT (Visual Head Impulse Test), fHIT. I risultati ottenuti sono incoraggianti e riportano come il danno vestibolare latente sia evidenziabile con tutte le metodiche con una percentuale compresa tra il 70 e l’85% nei soggetti OSA. Gli studi in essere stanno ampliando la casistica dei soggetti OSA vs controlli sottoposti a screening vestibolare completo (vHIT, fHIT e VEMPS) e si sta provvedendo ad integrare con la valutazione audiologica dei medesimi soggetti. Infine, sono in essere studi di valutazione della funzione vestibolare dopo trattamento delle apnee ostruttive del sonno (CPAP, terapia chirurgica).
