Thesis title: Pitfalls e falsi positivi nella diagnosi dermatoscopica di melanoma: studio osservazione retrospettivo.

Dermoscopy represents an indispensable first-line diagnostic tool in the assessment of melanoma, allowing, in experienced hands, an improvement in diagnostic accuracy by 10-30% compared with clinical examination alone. The principal dermoscopic diagnostic algorithms are designed to prioritize sensitivity over specificity; however, the increasing demand for second- and third-level diagnostic investigations risks exceeding the capacity to perform them within appropriate timeframes. It is therefore imperative to identify the factors that limit diagnostic specificity, with the aim of minimizing they impact and reducing the number of unnecessary follow-up examinations without increasing the rate of false negatives. The objective of the present study is to analyze the clinical and dermoscopic characteristics of melanocytic and non-melanocytic skin lesions that were clinically suggestive of melanoma but subsequently proved to be false positives on histopathological examination. The purpose is to identify the most frequent pitfalls limiting specificity and to compare the performance of different diagnostic algorithms and pattern analysis in correctly identifying true negatives in diagnostically challenging cases.
