Titolo della tesi: IMPLEMENTAZIONE DELLA DIAGNOSTICA NON INVASIVA A SUPPORTO DELLA PRATICA CLINICA

ABSTRACT L’uso di strumenti, intesi in senso lato come tutti quegli oggetti attraverso i quali possiamo interagire con l’ambiente e modificarlo, fa parte della nostra esperienza quotidiana. Ad esempio, quando usiamo uno smartphone per comprare un biglietto ferroviario stiamo di fatto agendo sull’ambiente, causando conseguenze tangibili attraverso uno strumento. Nella specie umana l’uso di strumenti è il risultato di un processo cognitivo complesso, che richiede di rappresentare mentalmente la possibilità di raggiungere uno scopo in maniera mediata piuttosto che diretta. Il legame tra strumenti e sviluppo della specie umana è centrale. Attraverso lo studio degli strumenti che i diversi ominidi sono stati in grado di produrre ed utilizzare, i paleoantropologi hanno da sempre cercato di caratterizzare i passi evolutivi che hanno portato all’emergere della nostra specie e delle nostre capacità cognitive. Questi primi utensili assumono, col tempo, forme sempre più elaborate e il cervello segue, a sua volta, un percorso evolutivo che porta a un aumento considerevole di dimensioni. Di fatto si può parlare, per questa particolare interazione, di un processo di co-evoluzione. Esaminando parallelamente la storia della medicina si evince come anche l’evoluzione di quest’ultima vada di pari passo al progresso tecnologico e di conseguenza delle strumentazioni a suo supporto. Le nuove tecnologie hanno di fatto trasformato la medicina e l’imminente avvento dell’intelligenza artificiale sarà l’elemento in grado di stravolgerla e conferirle una spinta evolutiva senza precedenti, fornendo di fatto il trait d’union tra strumenti tangibili e quelli astratti. In campo dermatologico, per tecniche diagnostiche non invasive ci si riferisce a un gruppo di metodiche in vivo che si affiancano alla valutazione clinica del paziente e che adoperano apparecchiature ad alta tecnologia per lo studio e la diagnosi di patologie cutanee, sia di natura infiammatoria che neoplastica. Questa tesi prende in esame alcune tra le strumentazioni diagnostiche più moderne ed innovative in Dermatologia, al fine di mostrarne le potenzialità ed i benefici nel delicato e basilare “momento diagnostico” e nella validazione dei risultati ottenuti dagli studi di laboratorio in applicazioni cliniche, tipico esempio di ricerca translazionale. ------ The use of tools, intended broadly as all those objects through which we can interact with the environment and modify it, is part of our daily experience. For example, when we use a smartphone to buy a train ticket we are in fact acting on the environment, causing tangible consequences through a tool. In the human species, the use of tools is the result of a complex cognitive process, which requires mentally representing the possibility of achieving a goal in a mediated, rather than direct, manner. The link between tools and the development of the human species is central. By studying the tools that the different hominids have been able to produce and use, paleoanthropologists have always tried to characterize the evolutionary steps which led to the origin of our species and our cognitive abilities. Over time, these first tools take on more and more elaborate shapes, and the brain, in turn, follows an evolutionary path that leads to a considerable increase in size. In fact, for this particular interaction, we can speak of a process of co-evolution. By examining the history of medicine at the same time, it is clear that the evolution of the latter also goes hand in hand with technological progress and consequently with the instruments supporting it. New technologies have in fact transformed medicine and the imminent advent of artificial intelligence will be the element capable of upsetting and giving it an unprecedented evolutionary drive, effectively providing the link between tangible and abstract tools. In Dermatology, non-invasive diagnostic techniques refer to a group of in-vivo methods that go hand in hand with the clinical evaluation of the patient and that use high-tech equipment for the study and diagnosis of skin diseases, both of an inflammatory and neoplastic nature. This thesis examines some of the most modern and innovative diagnostic equipment in Dermatology, in order to show their potential and benefits in the delicate and fundamental "diagnostic moment" and in the validation of the results obtained from laboratory studies in clinical applications, a typical example of translational research.
