Titolo della tesi: Prevention of Mother-to-Child transmission of HIV in resource limited settings: Determinants of mother-child health in three regions of Cameroon

Prevention of mother-to-child transmission of HIV remains a serious public health concern in Cameroon, especially in a context of multiculturalism and diversity of sexual behaviors in the ten regions. The aim was highlighting the determinants affecting mother-child health in Cameroon. To do this, we opted for the evaluation of the adherence to antiretroviral treatment of HIV positive women of childbearing age and the evaluation of the nutritional status and growth of their (exposed) children from birth to the age of six months, compared to those of infants born from HIV-negative mothers (not exposed). This was a descriptive and analytical cross-sectional study with a combined cross-sectional study. The cross-sectional part consisted of evaluating the adherence to antiretroviral treatment of HIV positive women of childbearing age, and affiliated to the care units of the regional hospitals of Bafoussam, Garoua, and Ebolowa; then Dschang and Ambam District Hospitals; belonging to the regions of West, North and South Cameroon. This assessment was carried out using a semi-opened questionnaire administered face to face and the stepwise analysis made it possible to highlight the determinants of adherence to antiretroviral therapy. The case-control part consisted in following two groups of children over the first six months of life. Children born from HIV-positive mothers (exposed) during the period of our investigation were matched by sex and age to those born from HIV-negative mothers (non-exposed) identified in the maternity units of the aforementioned hospitals. A form for anthropometric parameter’s follow-up (weight, height, MUAC, etc.) was used to provide information on the children's growth parameters. The informed consent of the women and that of the children’s parents/guardians were obtained before any investigation. Data collected was processed and analyzed using Excel 2010, STATA version 7.8 and ENA 2006 software, then presented with a significance level set at 0.05. A total of 1500 women equally distributed in the West, South and North Regions of Cameroon participated in the assessment of adherence to ART. According to age, women participating in this study were relatively young ([12 - 34] years) with an age mean of 17 ±2 SD. Overall, the prevalence of ART-adherence was 68.47% (1027/1500). After multivariate analysis, taking ART more than once daily (p <0.0001; OR = 0.009 [0.003-0.030]), non-disclosure of ART with the spouse (p <0.0001; OR = 0.001 [<0.001-0.005]), having only one dependent child (p <0.0001; OR = 0.019 [0.005-0.078]) and being a widow (p <0.0001; OR = 0.052 [0.013-0.214]) were all risk factors for non-adherence; On the other hand, having at least two dependent children (p = 0.0067; OR = 6.69 [1.694-26.423]), positively influences adherence to ART. In addition, growth monitoring was carried out on 339 exposed children and 340 non-exposed children. According to the weight-for-age z-score (WAZ) and global weight, the exposed children had lower growth rate than the unexposed children. ART-adherence (68.47%) among these Cameroonian women of childbearing age is still very far below UNAIDS goal (95%), with five potential key determinants identified. Children exposed to HIV grow slightly less than children not exposed to HIV. As we are progressively moving towards the elimination of HIV vertical transmission in 2030 as advocated by UNAIDS, local policies should be reinforced in order to consolidate the strides made so far.
